1. Enrollment contract.
2. Permission slip for trips.
3. Authorization form for children to be picked up by other family members
and adults.
4. Medical form to be filled out by the child’s physician and returned
to school by September 1. Children cannot be admitted without necessary
immunizations.
5. Emergency card.
